Prognostic prediction for moderate/severe bronchopulmonary dysplasia (BPD) by postmenstrual age (PMA) 36 weeks

This application is used for an infant admitted to neonatal intensive care unit (NICU). Two questions should be answered yes as an indication to use this application. First, is the patient's age 28 days after birth? Second, is this application used for the first time in this NICU admission?
